  • We help you organize your healtcare, from providers to medications. If you decide to share that with caregivers and family members, we can do that, too.

    We’re not your doctor or your hospital.
    We don’t work for your healthcare provider, and we’re not part of their medical record system. When you connect your records (such as through Epic), you’re choosing to share that information directly with us for your personal use.

    Our app helps you collect and understand your health information in one place. We only access your data when you give us permission, and we only use it to help you.

    We don’t sell your data. Ever.
    Your health information is not sold or shared for advertising.

    You choose who else can see your information.
    If you share information with a caregiver or family member, that only happens because you asked us to.

    We use AI to help explain things.
    We use AI to summarize visits, explain medical terms, and help you understand costs and insurance. AI helps you understand your care — it does not replace your doctor.

    You can disconnect or delete your data at any time.
    You can stop access to your health records or delete your account whenever you want.

    We take security seriously.
    Your information is encrypted and protected using industry-standard security practices.

    WellsbyMD (“we,” “us,” or “our”) provides an AI-assisted patient copilot application that helps users aggregate, understand, and manage their personal health information (“PHI”). Your privacy and control over your health data are central to our mission.

    • Notes, preferences, and user-entered health information

    3.2 Health Information from Connected Providers

    With your explicit authorization, we may access:

    • Medical conditions, diagnoses, and problem lists

    • Medications and allergies

    • Lab results and imaging reports

    • Visit summaries and clinical notes

    • Insurance and cost-related data (when available)

    All access is performed using FHIR-standard APIs and only for the scopes you approve.

    3.3 Automatically Collected Information

    • Device type, operating system, and app version

    • Log data (IP address, timestamps, error logs)

    • Usage analytics (feature interactions, session duration)

    4. How We Use Your Information

    We use your information to:

    • Aggregate and display your longitudinal health record

    • Generate AI-assisted summaries and explanations

    • Help estimate healthcare costs and insurance coverage

    • Provide visit summaries and caregiver-friendly explanations

    • Enable secure sharing with caregivers or family members you choose

    • Improve app performance, safety, and usability

    We do not sell your personal or health information.

    5. AI and Automated Processing

    Our application uses artificial intelligence to:

    • Summarize medical information in patient-friendly language

    • Identify patterns or reminders relevant to your care

    • Assist with cost and insurance understanding

    AI outputs are assistive only and do not replace medical advice. You remain responsible for healthcare decisions made with your providers.

    6. Data Sharing and Disclosure

    6.1 With Your Consent

    We share data:

    • With caregivers or family members only when you explicitly authorize

    • With healthcare providers if you initiate sharing

    6.2 Service Providers

    We may share limited data with trusted vendors who help us operate the app (e.g., cloud hosting, analytics), under strict confidentiality and security obligations.

    6.3 Legal Requirements

    We may disclose information if required by law, court order, or to protect safety and security.

    7. Epic FHIR & SMART on FHIR Compliance

    When you connect your Epic account:

    • We access data only through Epic-approved FHIR endpoints

    • We request minimum necessary scopes

    • You may revoke access at any time via Epic or within our app

    • We comply with Epic’s app usage, security, and data handling requirements

    We do not retain data beyond what is necessary to provide our services or beyond your authorization.

    8. Data Storage and Security

    We use industry-standard safeguards including:

    • Encryption in transit and at rest

    • Role-based access controls

    • Secure key and token storage

    • Regular security monitoring and audits

    Despite these measures, no system is 100% secure.

    9. Data Retention

    We retain your data:

    • For as long as your account is active

    • As needed to provide services you request

    • As required by law or contractual obligations

    You may request deletion of your data at any time (see Section 11).

    10. Your Rights and Choices

    Depending on your jurisdiction, you may have the right to:

    • Access your data

    • Correct inaccurate data

    • Download your data

    • Delete your account and associated data

    11. Account Deletion and Data Removal

    You may delete your account by:

    Upon deletion, we will:

    • Revoke EHR access tokens

    • Delete stored health data within a reasonable timeframe

    • Retain only data required for legal compliance

    12. Children’s Privacy

    Our app is not intended for children under 13. We do not knowingly collect data from children without verified parental consent.

    3. Not Medical Advice

    Information provided by the App, including AI-generated content:

    • Is for informational and educational purposes only

    • Does not replace professional medical advice, diagnosis, or treatment

    • Should not be used for medical decision-making without consulting a qualified healthcare professional

    Always seek the advice of your doctor or other qualified provider with any questions about your health.

    4. Your Account and Responsibilities

    You are responsible for:

    • Providing accurate and up-to-date information

    • Keeping your account credentials secure

    • All activity that occurs under your account

    • Ensuring that anyone you authorize (e.g., caregivers) is appropriate

    You must be at least 18 years old (or the age of majority in your jurisdiction) to use the App unless otherwise permitted by law.

    5. Connecting to Health Records (Epic & FHIR)

    When you connect your health records:

    • You authorize us to access your data through secure FHIR-based APIs

    • Access is limited to the permissions you approve

    • You may revoke access at any time through the App or your provider’s system

    We access your data at your direction and for your personal use. We are not acting on behalf of your healthcare provider unless explicitly stated otherwise.

    6. AI Features

    The App uses artificial intelligence to:

    • Summarize visits and records

    • Explain medical terms in plain language

    • Assist with understanding costs and insurance

    AI outputs may be incomplete or inaccurate. You agree to use AI-generated content responsibly and not rely on it as a substitute for professional advice.

    7. Sharing with Caregivers or Family

    If you choose to share information:

    • You control who receives access

    • You may revoke access at any time

    • You are responsible for the consequences of sharing

    We are not responsible for how others use information you choose to share with them.

    8. Privacy

    Your use of the App is governed by our Privacy Policy, which explains how we collect, use, and protect your information. By using the App, you agree to the Privacy Policy.

    9. Prohibited Uses

    You agree not to:

    • Use the App for unlawful purposes

    • Attempt to access another user’s data without authorization

    • Interfere with the App’s security or operation

    • Your misuse of the App

    • Your violation of these Terms

    • Your sharing of information with others

    Continued use after changes means you accept the updated Terms.
